we dont say what in their hearts but WE DO say what we see. so if u see a kafir praying to a statue, do you say may be he is muslim in his heart? heck no, you say he is kafir.
when some one says kalima then we say yes he is muslim but if he goes and starts worshipping the dead he no longer is following the kalima he just recited which states that there is no ilah but allah, and he becomes kafir.
