I just sent some of my students there and we had to go through a whole visa process through the Italian embassy in NY. Or maybe that was because they will be studying there? Hm. I guess just to visit is probably free.

Not sure, but you're probably right, though I had some friends, silly high schoolers  who went to Germany as exchange students for a month, didnt need a visa though...

Been to London several times and have lived there--you dont need a visa if on vacation. They will stamp your passport at Heathrow.
But if you decide to go abroad then you will have to apply for a visa
As for tickets--rates are different and it all depends. Usually found good rates from American Airlines and British Airways.

What about Green card holders with a Pakistani passport?

You will have to exit the plane in a parachute just before entering UK territory. The parachute must be of green color.
