Adding half a bottle of what I thought was soy sauce to a chicken stir-fry. Turned out to be Sesame oil. TRULY THE MOST DISGUSTING SUBSTANCE ON EARTH IF U USE MORE THAN 1 DROP.
One must make a mark in this world, if I am remembered for anything, let it be cheese and crackers...

Sugar in chicken karahi, instead of salt.
One afternoon when cooking paaye, added penalty of water and went out with friends. Came back after 4 hrs only to find out that I forgot to turn the hob ON.

Oh I can do anything.  I can even burn an egg when BOILING it. (did once)
Salt and sugar were in identical plastic jars. naak: it tasted THAI.

it happened twice. I put spoon full of salt in chai, instead of sugar ..... realized at 1st sip
I once bought half pumpkin thinking it was cantaloupe.

Occasion: First day of my wife with me in US as she flown in from Pakistan. (due to her documents problem she joind me couple of months later)
She was tired and changing/freshening up when I decided to make tea for her. By mistake I put 1 tspn sugar in her cup twice instead of 1 in each cup.
She started to take T and i was like "Chaiee sahee banee hai?"
She (most probably does not want to disappoint me): "yeah XXXX theek hai ..aap itnee HEE sugar laiteey hain chaiee main?"
Me: (no idea yet whats wrong as I still have to take first sip of my T): yeah itnee hee laita hoon Ziada sugar is not good anyway. Bus eik chaiee banana hee tu seekha hai main nai
Later on when i took 1 sip of my T i figured it out and she was like "naah... chaiee banana bhee nahee seekha"
She still embarrass me occasionally repeating the incident. 
jisey hum sai milna, gawara nahee hai, hum sai mila na karey.....

I've done the same thing many a time... but i use my dimaagh and put the iron on a reasonably low heat!

I don't have a story of my own mistake from the kitchen but I have one from a friend......
We've invited a few couples over and I'm running late to change clothes cuz the apetizer (i.e. fish pakoray) needs to be done last minute and I won't deep fry after I have changed.....so.......friend offers to help fry while I go and freshen up.
Thankful for the help I accept her gracious offer and show her the fish pieces in one bowl, masala and besan in another. I demonstrate by taking 5 or 6 pieces at a time, dipping them and lowering them into the karahi for deep frying. I show her when to turn and when to remove from oil and drain.
I disappear for 10/15 minutes returning to find ALL of the fish pieces sitting in the bottom of the oil filled karahi slowly bubbling away like a molten mass. The masala-fied besan and fish have turned into a yellow pulp.
My generous friend reacts to the horror on my face with, "I've never fried fish before. I don't know why it's not cooking like the pieces you did."
