What Islam says about someone who breaks/hurts someone's heart?
What punishment does such a person get?Does he/she get punished for this in this world itself?

Musalman woh hey jiskey haath aor zaban sey dosrey muslaman makhfooz rahein(Hadith). There many Hadiths and Ayats not to hurt Muslims. But if you are talking about Love Affairs than it is different matter.

Hmmmmm! Well my personal opinion, not an Islamic point of view..... When any one (male/female) has secret love affairs with somebody...... Not even bringing the family into the matter..... Do whatever they want..... and then ultimately as usual leave each other..... Because their affair is only based on sex ( sorry to say that)......

I think I made a thread about it with Islamic scriptures in it.
Anyways, if you are wronged: - your sins are expatiated just for the feeling of sorrow;
- you may forgive the other person and receive forgiveness for some of your own sins (which I believe is greater in calibre than point #3), and;
- you may not forgive the other person and have equivalent good deeds of his transferred to you, at the Day of Judgement.
It is, however, better if one forgives another. The best time to forgive is when angry. These are the works of true Muslims: conquest of self (the hardest thing to do in life).
...The nations which were before you were destroyed (by Allah) because they differed.
Sahih Bukhari, Volume 6, Book 61, Number 582:
